How to Fish leech quest: find 3 Leeches and get the bait
If the counter says 0/3, stop fishing. Leeches are ground pickups on
Island 2, not fish you catch with a rod.
Common mistake: older guides mention five Leeches.
Current community reports repeatedly show a 0/3 request: pick up
exactly three, return them to the NPC, and receive the modified
bait.

FAST ROUTE
Walk the grass, not the water.
Start near the woman by the lake and sweep the nearby grass, shore
and forest floor. Pick up the dark wriggling objects with your
interact key.
Talk to the NPC first.
Watch the counter reach 3/3.
Return before using the bait.

Step 1: start the request
Talk to the woman near the lake/settlement on Island 2. Confirm
the objective reads 0/3 before you start searching. If the counter
does not appear, progress the previous island quest first.
Step 2: collect three ground Leeches
Search the tall grass and damp shoreline around the lake. These
are pickups, so the rod, bait menu and fish pool are irrelevant.
Walk slowly enough to see the interact prompt and use your current
interact key.
Step 3: turn them in
Return to the same NPC with all three. The reward is the
leech-based bait used to summon the Giant Piranha. Do not sell or
cook the quest item.
Step 4: prepare before casting
Upgrade the shotgun if possible, keep cooked fish in spare
inventory slots, and clear a route around trees. The boss spawns
smaller piranhas, which can become emergency healing when the
fight gets messy.

Start at the quest giver and walk the nearest grass line first.
Keep the camera low; the dark pickup is easy to miss beside the
water.

Follow the shoreline, then cut through the tall grass and tree
roots. Leeches are ground pickups, so bait rarity and fishing time
do not change this route.
Do not cast while searching

Return and lock the reward
Once the counter reads 3/3, go straight back to the same NPC. Keep
the modified leech bait in your inventory until the Giant Piranha
encounter is ready.
Do not sell or cook it
Pack food before casting
Still stuck at 0/3? Leave the island and return
after advancing the previous quest. The counter may not appear until
the NPC dialogue has been completed in order.
